I've heard that the x-ray can be replaced with a tb test?

If it's anything like previous years, you can contact the consulate and get it clarified with authority.
Ostensibly the chest X-ray is to check for TB and some people who have extreme phobias of x-rays have had TB tests (IIRC blood TB tests) instead. (As an aside, you'll get more x-ray exposure from a flight from Aus to Japan than a single chest x-ray anyway.)
I'm actually drafting an email to the Melbourne consulate now.
I don't have a problem with being x rayed. My doctor was just unsure if it was necessary - along with his statement. Saying that he's not an MD but part of a fellowship.
So no drama doing what have to - just trying to clarify what that is.

Not an MD, just in a fellowship? I'm starting to get a cult vibe.
Seriously though, if you have no problem with an x-ray, just get an x-ray. It's a big part of any health check in Japan.
